Make sure that you have scheduled your meeting to include registration.
1. Once you have scheduled your Zoom meeting with registration, scroll to the bottom of the page where your registration options are listed.

2. In registration options click Edit.

3. In the pop-out winder click Custom Questions.

4. Click the New Question button to add a custom question to your meeting.

5. Choose your question type, add your Question, and click Create.

6. When you have created all of your custom questions click Save All.
