2. Click the Canvas icon in the top-right corner.
3. Click Dashboard in the Global Navigation Menu.
4. Click a course card to access that course.
1. Click Quizzes in the Course Navigation Menu.
2. Click the Add Quiz button.
3. In the Details tab, enter the name of your survey [1]. In the Rich Content Editor [2], introduce your survey with formatted text, images, video, or sample math equations.
4. In the Quiz Type drop-down menu, select the type of survey you would like to create.
5. Complete all the quiz options.
5.1. Place your survey in an assignment group.
5.2. Assign your survey a score.
5.3. Complete the survey options.
5.4. You can also keep submissions anonymous. This anonymous option applies to both graded and ungraded surveys and can be enabled and disabled before or after survey submissions.
6. You can also set a due date and available from/until dates for your survey.
7. Click the Questions tab. Click the New Question button to manually create a new survey question.
8. Survey questions are not automatically numbered for instructors. To add a custom name to your survey question, enter the name in the question text field.
Custom names can help you identify survey questions more easily. Regardless of the question name, students always see survey questions in numerical order (i.e. Question 1, Question 2).
9. Enter your survey question.
10. To save your question, click Update Question.
11. To create a draft of your survey, click Save.
If you are ready to publish your survey, click Save & Publish.
12. When your survey is saved in a draft state, you can return to the page and publish it at any time by clicking the Publish button.
The button will change from gray to green.
