Create a Google Form with a File Upload

1. In the TC Portal Click Google Drive

If you do not have Google Drive Pinned. Click More Applications.

2. In the top right corner click the grid icon.

3. Click Forms

4. In the forms Dashboard click Blank Form

5. Create a Title for your form and add any other fields you may want for your form.

6. To add the file submission click the add question option and select file upload from the dropdown list

7. To set up your file submission click continue

File submissions will be uploaded to the owner of the forms google drive in a folder.

8. Set up your preferences for the file upload.

9. Before publishing your form check the form settings to ensure everything is set to your liking.

In the settings it is recommended that you collect email addresses of the responders so you know who is submitting files to your form and what email they are using.

10. When you are ready to send your form out click Publish.

11. When you click Publish you will be prompted to select who can access this form. Click Publish.

It is recommended that you keep the security setting that users need to sign into their tc.columbia.edu email to submit files.
