Qualtrics can be used to create and distribute an online research consent form. The form is created as one question and the signature will require a second question. Respondent may sign using their computer mouse, finger, or stylus pen.
1. After adding the consent form, click + Add a new question. In the menu, click Signature.
2. Confirm that the signature question appears below the description of the study.
