Create a Form Field to collect respondent information in Qualtrics

You can create a question that has different fields to collect respondent information such as name, date, email, etc.

2. Click the drop-down arrow next to Text Entry in the left menu and select Form field.

3. The default for Form fields is 3 choices or fields, you can change this by changing the Number of  fields in the left menu.

4. To edit the question and options, click on the texts to edit.

4.1. You can edit the form field by clicking Click here to edit form fields.

4.2. You can choose how long you want a response to be (short, medium, long, essay) for each entry.

5. You can set a validation for each of your entry by clicking on the drop-down menu under Validation.
